ABSTRACT

Background: As one of the most common malignant tumors of the digestive tract, esophageal squamous cell carcinoma (ESCC) is an advanced metastatic cancer with an extremely high mortality rate and the highest prevalence rate in China. Spindle- and kinetochore-associated protein 1 (SKA1), an essential member involved in chromosome separation during mitosis, has been indicated as a potential biomarker in the pathogenesis and development of various types of malignant tumors; however, the exact functions of SKA1 in ESCC are still unclear. Patients and Methods: SKA1 expression was explored in stage IIA ESCC and corresponding healthy esophageal mucosa tissues through immunohistochemistry and reverse transcription–quantitative polymerase chain reaction and was further validated using The Cancer Genome Atlas (TCGA) database of the online tool UALCAN. Then, the clinicopathological correlations of SKA1 were analyzed based on the follow-up data. Furthermore, using the online tool LinkedOmics, the correlation test, gene ontology (GO), and Kyoto Encyclopedia of Genes and Genomes pathway enrichment analysis of SKA1 were analyzed using high-throughput sequencing data of ESCC patients from TCGA dataset. Results: The expression level of SKA1 was markedly upregulated in ESCC tissues. Upregulation of SKA1 significantly correlated with higher pathological T stage (P = 0.003) and poorer overall survival (P = 0.013). GO and pathway enrichment analyses of SKA1 in ESCC revealed that SKA1 was involved in a number of classical cell cycle-related pathways that contribute to special biological processes in tumorigenesis and development of ESCC. Conclusion: The results of this study demonstrate that SKA1 may act as a prognostic biomarker for stage IIA ESCC. Combined with the bioinformatic analysis, SKA1 could potentially serve as a therapeutic target for ESCC. Conclusion: The results coming from the present study demonstrated that SKA1 may act as a prognostic biomarker for stage IIA ESCC. ABSTRACT

Objective:To explore the clinical utility of tongue pressure resistance feedback training in the treatment of post-stroke dysphagia.Methods:Twenty stroke survivors with dysphagia were randomly divided into an experimental group and a control group. Both groups were given conventional swallowing rehabilitation training, while the experimental group was additionally provided with tongue pressure resistance feedback training. Before and after the treatment, MBSImp analysis and the Rosenbek penetration aspiration scale were used to quantify the control of the tongue, food delivery, oral residue, laryngeal elevation, hyoid bone movement, epiglottis turnover, larynx closure, vestibular larynx closure, pharyngal peristalsis and contraction, opening of the upper esophageal sphincter UES, contraction of the base of the tongue, pharyngeal residue and aspiration.Results:No significant differences were observed between the two groups before the intervention. Afterward the average pharyngeal period and aspiration score of the experimental group had decreased significantly compared with the control group′s values.Conclusions:Tongue pressure resistance feedback training is effective in improving pharyngeal swallowing and reducing the risk of aspiration after swallowing.

ABSTRACT

Objective@#To summarize and analyze the clinical data of hepatic venous pressure gradient (HVPG) and to explore the application value of HVPG in the diagnosis, evaluation and clinical treatment of portal hypertension in cirrhosis.@*Methods@#The patient data of HVPG measurement performed in Shandong Provincial Hospital from April 2010 to November 2017 were collected.@*Results@#A total of 633 patients with 833 times of HVPG measurements were included. There was significant difference in HVPG between patients with different etiologies, different Child-pugh grades and different degrees of decompensated cirrhosis.@*Conclusion@#The HVPG test is suitable for the diagnosis and evaluation of portal hypertension. The HVPG of patients with different severity of liver cirrhosis can guide the choice of the treatment plan, and the HVPG measurement should also be strictly standardized and quality control.

ABSTRACT

Objective To retrospectively analyze the effect of Brighton Pediatric Early Warning Score (Brighton PEWS) on the disease identification of children with hand-foot-mouth disease(HFMD). Methods Collected the hospital medical records of the children from January 2016 to July 2017 in our hospital, then compared the identification of the Brighton PEWS with the real outcomes, and analyzed the prediction effect of the Brighton PEWS by plotting the ROC curve. Results The study retrospectively analyze 619 cases of medical records, The area under the ROC curve of the Brighton PEWS was 0.838 (P<0.01), when the cut-off was 1.5, the maximum Youden value was 0.532, the sensitivity was 63.4%, and the identification was 89.8%. Conclusions The Brighton PEWS had a good identification when the cut-off was 1.5, but the sensitivity was low, we need to improve the sensitivity of the Brighton PEWS by combining with the characteristics of HFMD.

ABSTRACT

Objective The overexpression lung cancer1( OLC1) protein is overexpressed in a variety of human tumors.The purpose of the present study is to determine whether increased expression of OLC1 is associat-ed with colorectal cancer.Methods OLC1 expression was assayed in 150 colorectal cancer tissues by immuno-histochemical staining(IHC).Multivariate and univariate analyses were performed to determine the association between OLC1 expression and prognosis.Results Immunohistochemical results revealed that 107 out of 150 colorectal cancer patients had increased levels of OLC1.OLC1 expression was significantly correlated with UICC stage(P<0.001)and histological differentiation(P<0.001)in colorectal cancer patients.The 5-year overall survival( OS) rates in patients with strong positive and less OLC1 staining were 16.6%and 95.3%,respectively (P<0.0001).Conclusion OLC1 overexpression is an important factor in colorectal carcinoma prognosis and can be an interesting potential novel biomarker for colorectal cancer.

ABSTRACT

Objective To investigate the association between serum sex hormone levels as well as body mass index and breast cancer in postmenopausal women .Methods We selected the cases of postmenopausal women who accepted surgical treatment for the first time which from March 2013 to December 2013 in the depart-ment of breast surgery the Affiliated Tumor Hospital of Harbin Medical University ,of which 118 cases of patients with breast cancer and 60 cases of benign breast lesions as control group .Meanwhile,information of body height and weight.were collected.Serum estradiol(E2),estrone(E1),testosterone(TSTO)and androstenedione(AED) concentrations were measured in 118 postmenopausal women with breast cancer patients and 60 matched control subjects by enzyme-linked immunosorbent assay(ELISA),then the results were estimated.Results The levels of serum E2,E1,AED of breast cancer group were significantly higher than those in control group (P0.05).The levels of body mass index(BMI)were not statistically significant(P>0.05)between breast cancer group and the control group .The level of E1 in breast cancer group and levels of E 1,TSTO,AED in overall of overweight group were significantly higher in overweight (P<0.05),yet we have not found association between levels of hormone and BMI .Conclusion The level of serum sex hormones is higher with breast cancer in postmenopausal women ,while high serum levels of E2,E1,AED maybe associated with breast cancer in postm-enopausal women .Sex hormones are higher in postmenopausal women with high BMI .

ABSTRACT

ObjectiveTo assess the effectiveness of tele-medicine and self-management goal(SMG) setting technique used in the diabetes management in the community setting.Methods It is a control-group study.415 type 2 diabetic residents were recruited from the Shanghai Wuliqiao community based on existing medical records.The subjects were divided into two groups,the study group was cared by general practitioners (GPs) specialists cooperation through the tele-medicine mechanism,the other was a control group.For the study group,a cooperation pathway between community health care centers and general hospitals were established.Standardized training and guidelines were provided to community health workers,regarding the setting of management goals of blood glucose and blood pressure,treatment plan,patient education,and SMG techniques.Fasting blood glucose ( FBG ) and 2 h postprandial blood glucose (2hBG) in the study group were monitored,followed by community health workers visiting monthly with seminars for diabetes education.At the baseline and the 12tb month,FBG,2hBG,HbA1C,blood pressure,triglyceride,total cholesterol,body mass index,waist-hip ratio were determined in each group.A survey was conducted to evaluate the costs of diabetes treatments,the knowledge base related to their disease,lifestyle,and the awareness of the new care model.The rates of achieving the goal of blood glucose,blood pressure,and HbA1Ccontrol were calculated.Internet case discussion between GPs-Specialists and referral to certain specialists were implemented when some patients did not reach the control goal.ResultsBy the 12 month follow up,FBG,2hBG,HbAIc,blood pressure of the study group were lower than the baseline,and as well as the control group with statical significance (P<0.05).There are other improvcments:diabetes knowledge (29.1% vs 5.5% ),healthy diet (9.6% vs -10.4% ),blood glucose monitoring (30.3% vs 10.8% ),support for diabetes care in community (35.7% vs 9.4% ),and the preference of the new model (63.8% vs 17.9% ) with statistical significauce (P<0.01 ).As for the medical costs,the study group's monthly costs were consistently lower than the control's.( -3.39Yuan vs 32.26 Yuan,P<0.05).ConclusionsThe new diabetes care model based on GPs-Specialists tele-medicine and SMG in community opens the door to the community based care model formulation in regard to the health quality and costs control.The deployment of more technologies and management techniques could be explored further to improve the outcomes of community based chronic disease care model.
